Customer Service Correspondence
If you send us correspondence, including faxes and emails, we retain such information in your account's records. We will also keep customer service correspondence and other correspondence from Michigan Integrative Health to you. We preserve these records to improve and measure our customer service. If permitted by law, we may, over time, delete these records.
Questionnaires, Profile Data and Surveys
We offer optional surveys and questionnaires to our users from time to time, for such purposes as assessing users' interests and needs or collecting demographic information. We explain in detail the use of the information collected in surveys. Before the participation in a study or a survey, we provide our users with information on how we use personally identifiable information if collected at all.

Internal Uses
Michigan Integrative Health uses the information collected about you for the following: to provide our services; provide customer service; determine your eligibility to use our services and products; and to improve our products and services.
Disclosure to Third Parties or Other Michigan Integrative Health Patients
Michigan Integrative Health will not sell or rent any of your personally identifiable information to third parties. Michigan Integrative Health will not share any of your personally identifiable information with third parties except in the limited circumstances described below this paragraph, and with your expressed permission.
1. Patients of Michigan Integrative Health who have proven successful in their weight loss may be invited to partake in our Testimonial Program. We don't compensate patients that participate in our Testimonial Program. Our patients' endorsements could include a before and after picture, name, pain area or conditions they overcame, and a personal statement. Reviews and testimonials are distributed to print media and electronic media, including the internet, television, and radio.
2. In good faith, we disclose information that we believe, is appropriate to cooperate in investigations of any illegal activity, including fraud.
3. We disclose information in response to a warrant, subpoena, court order, levy, attachment, an order of a court-appointed receiver or any other comparable legal processes, including subpoenas from private parties in a civil action.
4. We disclose information to your legal representative or agent (such as a guardian appointed for you or a person you granted power of attorney to).
5. We share aggregated statistical data for public relations or with our business partners. For example, we might disclose that a specific number or percentage of our users live in a particular region. However, this is only aggregated information and is not tied to any personally identifiable information.
6. As it always happens in the business world, it is possible that Michigan Integrative Health could merge or be acquired by another company in the future. If said acquisition occurs, the successor company would have access to the information maintained by Michigan Integrative Health. This information includes customer account information but would continue to be bound by this Privacy Policy unless and until it's amended as described in Section A above.
Our Contacts with Michigan Integrative Health ® Patients
Regularly, we communicate with patients via email to provide requested services, and we also interact by phone to service patients and resolve customer complaints. We use your electronic mail (email) address to confirm your Michigan Integrative Health appointments, to send information about any significant changes to our products and services, and to send notices and other disclosures required by law.
In regards to independent audits of our operations and financial statements, the auditors may seek to contact a small sample of our customers to confirm that our records are accurate. However, these auditors cannot use any of your personally identifiable information for any secondary purposes.

We use IP addresses, access times, and browser types to improve site performance, gather broad demographic information, analyze various trends, and administer the site only for aggregate use.
